Please note: The new proof of claim form B10 (Official Form 10)(12/11) is effective December 1, 2011.  The Trustee will be obliged to object to any proof of claim filed after December 1^st that is not on this new official form AND, in addition to ensuring that the form has the usual requisite information, the Trustee will object to any new form that is filed without  the box checked in section 8.
